Residency Verification Process
In accordance with the Fulton County Board policy, all 9th and 10th graders, new enrollees, or households with a change of address must submit proof of residency. To submit proof of residency:
- Please download and complete the Residency Verification Form at the link below.
- Provide two proofs of residency as outlined on the form and listed below.
- Please only send acceptable documents outlined by Fulton County. Do not send documentation without the Residency Verification Form.
Please email the completed Residency form with two proofs of residency to CAMHSresidency@fultonschools.org.
