11th Grade Meningitis Vaccine Requirement
Meningococcal Conjugate Vaccine (MCV4) Booster - Due September 30, 2024
For the 2024-2025 school year, all 11th-grade students must have an updated GA 3231 immunization certificate on file in order to remain active at Cambridge High School. Effective July 1, 2021, children 16 years of age and older, who are entering the 11th grade (including new entrants), must have received one booster dose of the meningococcal conjugate vaccine (MCV4) unless their initial dose was administered on or after their 16th birthday.
Note change: The Georgia Department of Public Health has developed a new Affidavit of Religious Objection to Immunization Form 2208 for use by all new students enrolling in a Fulton County School beginning with the 2015-2016 school year who wish to submit a religious exemption. You can find the new form at the Georgia Department of Public Health under forms and brochures.
GA 3231 immunization forms can be emailed to CAMHSimmunization@fultonschools.org, or left at Bridge West.

- Rising 7th Grade Students,
- New Entrants/Students Grades 8 - 12th and
- 11th Grade Students
11th Grade: All students entering or transferring into the 11th grade will need proof of a meningococcal booster (MCV4) unless their first dose was received on or after their 16th birthday. Meningococcal disease is a serious bacterial illness that affects the brain and the spinal cord. Meningitis can cause shock, coma, and death within hours of the first symptoms. To help protect your children and others from meningitis, Georgia law requires students to be vaccinated against this disease unless the child has an exemption.
